This case management review expect detailed evaluation of the provision and coordination of care for one older male client (80 years plus who has, cognitive impairment- mixed Alzheimer’s, 2nd Fall to Parkinsons, AF, HT, CCF, T2DM, Dyslipidemia, etc) with whom I have worked during my clinical placement. Word limits 2000 words.
There is an additional form Nursing Health Assessment Framework for collecting data, this is not included in the word count, that the reason I paid for extra 500 words.The expectation is still try and have the conversation with the patient. Cognitively impaired patient may still have vivid and reasonable accurate memories of childhood etc. The reality is that a lot of information will be dependent on records, staff, family members. Please just make sure in the paper as to where the information came from and why the selected patient was unable to speak directly to the interview.
